Combining both of the previous methods and spreading clusters to different geographical locations provides the highest level of reliability.  This set up not only protects against failure of servers and load balancers, it also protects against the possibility of network interruption at a datacenter location.

Advantages:  The highest level of failover protection; the ability to route traffic to the fastest relay for any given end user.

Disadvantages:  High cost; greater complexity involved in syncing data; latency in getting all servers synced.

This brief primer gives you a good background into what is involved in creating server clusters.  One thing we did not cover is how data is synced between servers.  There are several ways to do this and which method is chosen depends on a lot of variables.  Some methods are quite simple and utilize tools readily available within the OS, such as rsync for Linux.  Other methods require expensive software.  It all depends on the nature of the data being synced and whether or not there can be any latency.
